Othercide’s stark black-and-white visuals, with red accents, are lauded for their "Sin City"-esque gothic horror aesthetic and Lovecraftian enemy designs, creating a unique, atmospheric experience. However, critics panned the repetitive monochrome palette as bland and visually taxing, citing UI issues, character indistinction, and unoriginal environments that undermine its initial impact. While the art direction is praised for its boldness and thematic cohesion, technical flaws and design limitations detract from immersion, making it a striking but flawed visual experience.
